Another Flywheel Gone?

I have a 1993 Explorer that is not my daily driver. I was getting ready to do a tune up on her when the flywheel went out of if for the second time in about 15000 miles. Has anybody had or heard of this problem before? The failure happens around the center where it bolts in place. It has around 112000 miles on it.

Did the flywheel shake off or break?
 






It sheared off causing some damage to a bushing in the starter and a little damage to the torque converter (nothin major). It tore the center right out of it where the center would spin and the outer part would not.

The same exact thing happened to me twice now, I have a 93 XLT with 110,000 mile on it. It first happened back in april and I fixed it myself and then it did it again in october and I had to fix it again. I have been dropping the starter about once a month to check on it and it seems fine so far. I have been told that either my engine or my tranny could have some sort of vibration and that could be causing it, my tranny is starting to shift funny so im thinking of getting a rebuilt one soon and see if that fixes the problem. Good luck with it.
 






My transmission is acting up also. I is almost like it is slipping, but it only does it when I put it in gear, not under a load. I did find out that Ford has redesigned the flywheel twice since the first time mine went out.
