I think that you need to remove the two screws above the ashtray and then the radio before the panel comes loose. I think that the switch is screwed onto the back of the panel, but I haven't taken it apart myself.
Remove the two 7mm bolts from right above the A/C panel. Pop the plastic trim out all around the edge untl the metal retainers are free. Pull the trim (complete with radio and all switches) out enough to get your hand in the right side. Unplug the three connectors from the rear wiper, rear defrost and fog lamp switch assembly. Pull out a little more and remove the two philips screws that hold the switche assembly in. Use two small flat screwdivers to release the defrost switch. Once you have it out, you'll see the light bulb on the side that went towards the A/C panel. Here are the three bulbs I recently replaced on mine. The fog/defrost bulb is the '98 part number (F32Z-13466-H). It runs about $4.00 at Ford.
EDIT: I just read that the headlight switch part # is F57Z-13B765-AC. I haven't bought one so I can't confirm.
