Engine noises..??

Hmm... well I'm not sure if this is anything to be worried about but I thought I'd ask before it became something that was not able to be fixed without lots of money dumped into it..

First, I'm getting this sluggish start when the engine is cold. The odd part is that the engine almost stalls out for a moment...I'm not able to put the truck into gear and go, I have to wait a moment and let the engine smooth out before I do or it stalls out. This ONLY happens when I start it in the morning or when the engine has sat for a period and gets cold. During the day, when its warm, none of the problems occur. Any ideas?

Also...

Occasionally I will give some gas and it feels like i'm slipping out of gear...the engine rev's up but there is no power going to the wheels...if I let off the throttle for a second and then re apply it kicks in and I'm fine... sound like a transmission going? If thats it..what are my options?

..AND... last question. I have the 3.55 diff ratio and was thinking of going to 32 X 11.5" M/T's... I was told in another post that I should be able to drive with those for a period without much trouble for street use, BUT when I do go to the 4.56 that I want...or possibly the 4.10, do I need to change the T case out too?
