idle issue in a 2003 Ford Explorer XLT V8 4.6L

Hi. I'm having issues with my Explorer idling too high. When I start it up, it sometimes idles up into the 3k RPM region. When driving, it revs up and drops down repeatedly and when I come to a stop, it bucks and revs and finally jerks to a stop. I was having issues with it stalling, and my brother in law replaced both the IAC valve and the MAF sensor (because it threw those codes). Now instead of it idling too low, I have the opposite issue. It was really bad when they were first replaced - it maintained a steady 45 mph without me hitting the gas. I took it to my mechanic where they told me the problem was the air filter (I very nicely told him that was bull), and they took it back (as it obviously was not fixed) and they replaced the the throttle position sensor. It helped but didn't cure the problem. Both my mechanic and I are running out of ideas. I did read somewhere that a vacuum hose could be the issue, giving the engine too much air, so it takes in more gas. Is that true? I'm getting tired of sinking money into it.
