Rattling noise when shifting into R or D

The title says it all. The problem is in my 93. 2wd Sound like slippage combined with clunking and rattling. Sometimes you can feel it in the cab, sometimes not. It only does it immediatly after shifting into R or D, but a couple of times it did it at red lights. Fluid looks good and full. This does not happen all of the time, just a few times a day.

Any ideas?
 






I would suspect u-joints.. Check them for play.
 






Just Replaced 6 months ago. It really sounds internal. Thanks for the thought
 






Problem is torque converter (thought so), but it has sent graphite shards into the tranny and begun to wear away at components. This one will hurt!
