I may not be understanding you correctly, but if this is a rumbling noise underhood while the engine is running and if this noise seems to be coming from up front near the belt:

Try removing the belt to see if the noise goes away. If it does, then turn each belt-driven pulley to feel for any lack of smoothness (rumbliness) to the spin or wobble. If you feel anything there, that's probably the part that's gone bad.

If you felt nothing, try putting the bely back on and let the engine run. Place a stick or similar object against each likely suspect particularly the alternator, tensioner pulley, and idler pulley. Be very careful not to let your stick touch any moving parts as this can cause you to become injured. In the case of a pulley, put your stick against the bolt in the center which does not move. Listen for your noise to be transmitted through the stick.

If however the noise is still present with the belt off, recheck your exhaust. If the noise happens only in gear you may need to set the parking brake and have someone sit in the truck with it in gear while you listen for the noise. Sometimes you can listen through a piece of rubber hose with one end near your ear to help localize the noise.

Check the idler pulley, they go bad on these trucks. It is the lone pulley on top attached to the bracket. I would replace it no matter what, if it breaks you are stranded. They make a funny noise before they go.

Helo there,
Bud, I took of the drive belt & start the engine, engine sound was perfecr, (but just boiled up the water so quick) and checked all the pullies one by one, found belt tenssioner puly has funny bearing noise, changed & Its OK.

Thanks for every one to help, I do appreciate.

For prevention I would change the top idler one too. Mine went bad soon after the tensioner and I have a 96. I saw many others with this problem. It is like $10 and worth to do, if it breaks you overheat and get stranded.
