Tranny issues

Below are 2 previous threads concerning all the troubleshooting I've been doing. What all has been replaced:
Tranny rebuilt ~ 1 year ago; all 4 O2 sensors; MAF; TPS; A few years ago: plugs, wires, coil pack.
I've been unable to get forscan to work, trying again today, have been using Torque app so far.

Point & Questions: About 2 weeks or so before replacing the fuel pump the truck was acting worse, more stalling, more loss of power and missing shifts almost every drive. I had reset the ECU, removed power, shorted +/- to drain the caps. When I did the reset I ended up having a lot of highway driving after, which probably wasn't the best for re-learning. At one stretch on the highway it kept shifting in and out of O/D. Could watch the RPMs drop & come up by ~800 or so every 20 to 30 sec maybe. No drive ability issues, held the speed pretty steady. Turned on the cruise and the same situation. I reset the ECU again, after arriving at my destination. Then from there home was not highway. Seemed to be fine. The O/D flashes occasionally, no hard shifts, the tranny actually feels pretty good. My wife mainly drives it, she has said it seems to be reving high on the highway, like its not going into OD. I'm going to look over the tranny today for any loose connections etc. try to get forscan to work and drive it to see what I can see. I've been in touch with my mechanic that has rebuilt the tranny.

Any insight would be great.

Was able to sit down with my mechanic yesterday. He spoke with the tranny association (similar to ASE) and some research. What seems to have happened, for some reason the ECM commands incorrect shifts and begins to cause damage to the internals of the tranny. My mechanic is taking it in next week to go through it and try to determine why it was commanding bad shifts and what damage is in the tranny.
Any body have any insights or experience with this? I'm getting close to cutting my loses on this one and finding something else for my wife.
 






Just in case anyone was wondering. I got the truck back from the mechanic and my wife drove it today. There is an adjustment bolt that controls the intermediate band (2nd & 5th gear), this is accessible externally. It had come loose causing the tranny to miss or shift poorly into 2nd. He also dropped the pan and check for internal damage, thankfully there was none. Seems to be back in order, now to get tires.
