Weird Idle???

once in a while when i start the X the idle guage and the battery guage needles rev up and down along with the X itself. i have to give it some gas for a few seconds untuil it then evens back out. this does not happen on every startup, and it never happens once i start driving. it does it in park, and then gets worse once it goes in reverse or drive, until i actually get going on the road. what can this be?

i replaced the IAC about 2 years ago, and i remember the reason i knew it was the IAC was because it had a loud whining sound. but i don't have anything like that sound now. i guess it could still be that, but i'm not positive.

next time it's idling rough try to tap on the IAC with a wrench.
