1992 Explorer Transmission issues

I have a 1992 Ford Explorer 4x4 that starts off fine when its cold but after it warms up the transmission fails. Is this just a maintenance issue or does it have to be replaced?

A little more detail about the symptoms would help here. When you say "fails", describe what happens. Won't go into gear?
