body lifts and steering extension - when?

As far as I've heard a 2' lift is as far as you can go without the steering extension. I think that is the limit.
 






I can say that I have a 2 in sloped body lift and I dont have an extension, steering still feels the same.
 






I know a mechanic who just takes out the pinch bolt in the steering shaft, end of story.
I guess if the shaft slides 95% out, it needs an extension. I've never done it, but it's been done
