clarification of blend dorr and actuator

2002 explorer xlt 4x4
first my problem with my 2002 xplorer. my blower motor fan speed control would only work when on the #4 setting and the air only comes out of the vent. I replaced the blower motor resistor and that has fixes the blower motor fan speeds. Now I'm researching the forum for the fix for the air only coming out of the defrost vent. What I'm reading is:
1. possible vacuum hose loose
2. blend door is broke
3. blend door actuator is broke

I was hoping someone could verify if I understand the function of the blend door and actuator.

Is the blend door the piece that divert the air flow to the defrost vent to the vents on the dash to the vents on the floor? If I understand correctly, to repair the blend door the dash has to be removed and is a lengthly process. Am I correct?

Is the actuator the mechanism that moves the blend door? Does the dash also have to be removed to do this repair?

What is the best way to determine which is in need of repair? Thanks in advance.
