do i need a new tranny? please help!

hello,

ok heres the situation: ever since i've had my 92 explorer (a little over a year), it has had trouble shifting while running cold. it usually corrects itself after about 5 minutes of driving. last night, it would go to about 4000 rpms before finally shifting from first gear to the next. after that i was driving about 40 mph and was running about 3000 rpms. usually it will have shifted into the next gear by this time, but no matter what i could not get it to shift. i had to make a short trip on the highway, and was going about 55 mph at 4500 rpms! and still could not get it to shift. there was also an occasion earlier in the evening where i was slowing down for a right turn and as soon as i hit the throttle after making the turn, the back axle seemed like there was something keeping the wheels from making a smooth rotation. it was very "jumpy". immediately after that i noticed a huge decrease in power (it seemed to me that it was not downshifting itself to the lowest gear for low speeds). it felt like a kid's go-kart trying to start off on a very steep hill.
at this moment, besides the rough shifting, it is still running and driving. i'm just doing it a little bit more carefully. i'm hoping that i can catch whatever problem it is before it gets too much worse. has anybody experienced or heard of any problems like this? if so, what are my options at this point? obviously i know i need to get it to a tranny shop, but what sort of news can i expect to hear from them?
