Idles rough on a cold start

1999 Mercury Mountaineer
I have a 1999 Mercury Mountaineer (Exploder wannabe), 5.0 V8.

Lately I have been having intermittent cold start issues. I'd say on about 50-60% of cold starts it idles VERY rough and will often die, requiring a restart. The initial RPM when it does this is incredibly low. The rest of the time the RPM is fine on start up and there are no issues. If I have the time to sit there and rev the engine slightly for a minute or two, the idle will normalize.

Once the vehicle has been driving, usually as little as just a couple of blocks, the issues resolves. I can park, turn off the engine, restart, and it's just fine.

Any thoughts on where to start to troubleshoot this? At the moment, it's just annoying, but I'd rather fix it before / if it turns into something more serious.
