Further Investigation leads me to believe fuel pump is the real issue
I have been experiencing extreme OCD over this whole issue, trying to get to the root of the problem, rather than a work-a-round. A good friend suggested that I should drive over to the local Ford dealer, park in the back, throw them my key, and then walk to the front and drive out with a new one. LOL
I have come up with the following information I'd like to share:

1. When idling normally, the injectors are getting turned on for 4ms.
2. When the problem occurs, the pulse width goes up to as high as 12ms, which makes it start to smooth out, then slowly drop back to normal 4ms pulse width.
3. As an experiment, I did a hot soak while the fuel pump relay was jumped leaving the fuel pump running for the entire hot soak period, 15 minutes. The vehicle would start up and run perfectly normal ! I repeated this many times, and always experienced the same results.
This leads me to believe the fuel pump has enough pressure (already been there and done that many many times) but perhaps not enough flow to flush the heated fuel out of the fuel rail quickly enough. I have already changed the fuel filter.
Does anyone have any other suggestions ? I really don't want to spend $300 to have the pump replaced and still see the same problem.
