I'm not quite clear on what you mean, but my guess is that you don't need a special Ford part for this, that you just need to take the line to an auto parts store, or measure the nipples it attaches to, to get the inner diameter, then have them cut you the appropriate length of standard vac line hose. As for the Y shape, you'd put a vacuum T coupler on it, something like one of these, the correct size for the hose(s):

If yours already has a plastic T-coupler you may be able to reuse it but I'd get the lines off first to be sure they come off without breaking it, saving you a 2nd trip to the auto parts store although currently Autozone has free shipping with no minimum for website orders. However some at Autozone are ridiculously overpriced, the t-coupler should be closer to $3 than to $20. There are lots of sources closer to $3.
If you have trouble figuring out the diameter of hose you need, you can probably ask the parts counter person at the auto parts store to cut you a little free sample of hose, 2" long piece or whatever, to try for fit before buying a longer length.
