Where is the best place to run breather extension lines to?

Do I just slide a rubber hose over the end of the metal line and run it to wherever or is there more to it than that?

Where did you run the breather lines from the axles to?

Slid the new hose over the metal line and use a hose clamp. Rear axle run the line to the fuel filler line and zip tie it. Front line up into the engine compartment, and zip tie. Make sure you have adequate line length to allow suspension to cycle. Test drive when complete and check to make sure that the hoses have not bound and pulled loose.

Some people will run the lines into a small air filter like a crankcase vent.

Thats kinda what I did..

Actually, my rear diff line runs up the brake line then over to the frame. Down the Frame where it meets up with the breather for the xfer case and Transmission (auto). At this point I tie all of the lines together and run it more forward where the front diff breather hooks to it. Now I have a single line that runs up under the hood into a small k&n filter that is mounted to the fender.

I think I'm going to modify what I have a little since in theory it is possible for ATF to go from the Transmission/Xfercase into a Diff (I doubt the diff fluid would ever make it into the transmission/xfercase).

I think the change will be to combine the front/rear diff breathers and run that to a single filter and then do the same with the transmission/xfercase. This will keep the fluids the same between the devices so I don't have to worry about contamination when wheeling (so far I have found any ATF in the Front or Rear diff).

The metal breather line above the tranny has a small hole where the line bends at the bell housing (see pic), does this need to be covered?

Yes it does.. I forgot to mention that as I don't run a ford transmission on my X.. I have teh 700r4 conversion which just has a small tube coming out the top.

I stuff the rear breather behind the tail lights near where the trailer connector comes out.

All that needs extending is the front and rear diff breathers? And I guess that tranny hole?

I know the 1354 Xfercase has a breather too.. (which I extended).. I don't know if your xfer case has one too.

All transfer cases have to breath as the system warms up and cools down, so all of them have some sort of a breather :)
