On my last nerve and need advice

I built a smoke leak machine and replaced I believe all leaks that were present. Throttle body gasket, intake manifold o ring gaskets, iac gaskets as well as cleaned the iac extensively. MAF cleaned and also the leaking egr valve has been replaced. Reset PCM and drove about 25 miles after each repair but still cannot get rid of the rough idle. When at idle with the ac off physically looking at the engine it has a bad shake to it. With the ac on it improves the shaking and roughness at idle quite well. After all gaskets have been replaced it is now to the point that random cold starts will cause it to almost stall but corrects itself. Before it was every cold start. I'm at my nerves end. Any ideas what else??

Fuel injector, spark plug, compression test?
 






Upon another smoke test I just ran the throttle body position sensor has a leak. Could it cause that much problems?
 






Depends how bad the leak is. Good plug wires with good/properly gapped plugs?
CEL on?
 






Ok so I replaced the sensor and it made an incredible difference in the throttle response. It is great but unfortunately the shake is still there. I wasn't able to do the spark plugs myself about 2 years ago or 19000 miles because I was so busy at work so I cannot comment on the gap being correct. If I remember right it didn't seem to have that issue until recently so I have a feeling the plugs were gapped and put in correct. I might just have to sell it. Very frustrating. Thanks for the replys everyone. Keep the suggestions coming
 






The cel isn't on anymore. About 6 months ago my sister was in a bad wreck and I hopped in the truck and was driving it pretty hard to get there. The check engine flashed for a little then went away. About 3 months ago it came on how lean condition bank 1 and possibly 2. I cleared it. 2 weeks ago it said downstream o2 sensor circuit malfunction so I replaced that and since then no check engine light.
